English Medical Certificate for Medication Prescribed in Japan
For customs, travel and insurance claims. Issued by a licensed Japanese physician together with your online consultation — at no extra charge.
In short
- Issued for medication prescribed at Ikegawa Clinic after an English online consultation.
- Included in the consultation fee. No separate charge.
- States the medication, dose, quantity and treatment period, with the doctor's signature and the clinic seal.
- Does not state a diagnosis (self-pay treatment) and is not a government document.
Who This Is For
✈️ Carrying medication out of Japan
Customs officers may ask why you are carrying prescription medicine. The certificate explains, in English, what was prescribed and that it is for your own use. Keep it with the receipt and the original packaging.
🧾 Claiming from your insurer
Travel and private health insurers often ask for a doctor's document in English in addition to the receipt. The certificate and our itemised receipt cover this.
🩺 Showing your doctor at home
The certificate lists the brand, generic name, strength, quantity and treatment period, so the doctor who continues your care knows exactly what you received.
🗂 Your own records
A dated, numbered document from the prescribing clinic, with a contact address for verification.
What the Certificate States
The certificate is an A4 document in English with the following sections.
| Section | Content |
|---|---|
| Clinic | Ikegawa Clinic (Ehime, Japan), address, telephone, email, and the name of the director |
| Certificate number and date of issue | A unique number for verification |
| Patient | Full name as on your passport, date of birth, nationality, sex, passport number and passport dates |
| Prescription | Brand name, generic name, drug class, strength and form, quantity dispensed, how it is taken, treatment period, and the date of the consultation |
| Statement | That the medication was prescribed by the undersigned licensed physician after an appropriate online consultation, and is for the patient's own personal use within the scope of self-pay treatment in Japan |
| Physician | Dr. Masayuki Ikegawa, MD, board-certified internist and neurologist, with signature, clinic seal and Japanese medical licence number |
What it does not do
- It does not state a specific disease or diagnosis name, because the treatment is self-pay (private).
- It is not an official certificate issued by the Japanese government or any government agency.
- It is not a sick note for work or school, a fit-to-fly letter, or a medical examination report for a visa.
- It can only cover medication prescribed by Ikegawa Clinic. We cannot certify medication from another clinic or bought abroad.

- Keep the medication in its original packaging, and carry the certificate and the receipt with it.
- From Japan's side, no export permit is needed for prescription medication for personal use.
- Rules in your destination country vary widely. GLP-1 medicines such as Mounjaro and Wegovy are restricted or banned in some countries. Check with that country's embassy or customs in advance. A country list is on the pricing page.
- Injectables need refrigeration. Plan how you will keep them cold during the journey.
Frequently Asked Questions
Is the English medical certificate free?
Yes. It is included in the consultation fee for English online consultations (¥5,500 for Mounjaro or Wegovy; ¥11,000 for a first visit and ¥5,500 for a follow-up for other prescriptions). There is no separate charge for the certificate.
Can I get a medical certificate without a consultation?
No. Under Japan's Medical Practitioners' Act, a physician may issue a certificate only for a patient they have actually examined. Please book an online consultation; the certificate is issued together with your prescription.
Can you certify medication prescribed by another clinic or bought abroad?
No. We can only certify medication that Dr. Ikegawa prescribed to you after a consultation at Ikegawa Clinic. We cannot issue certificates for medication you obtained elsewhere.
Does the certificate state my diagnosis?
No. Because this is self-pay (private) treatment, the certificate does not list a specific disease or diagnosis name. It states that the medication was prescribed by a licensed physician after an appropriate consultation and is for your own personal use.
Is the certificate accepted at customs?
The certificate is the clinic's own original document. It is not a certificate issued by the Japanese government. Many travellers present it together with the receipt and the medication in its original packaging. Import rules differ by country, and GLP-1 medicines such as Mounjaro and Wegovy are restricted or banned in some countries, so please check with your destination country's embassy or customs before you travel. The clinic cannot take responsibility for confiscation at customs.
How and when do I receive the certificate?
As a PDF, attached to the receipt email we send once your payment is confirmed. If you need a paper original with the doctor's signature and the clinic seal, please tell us when you book.
Can I use it to claim from my travel or health insurance?
Usually yes, together with the itemised receipt we issue. Insurers have their own requirements, so please check with your insurer what documents they accept. We do not bill insurers directly.
