Online Medical Consultation in English

English-speaking video consultation via Google Meet. Weight-loss medication and general prescriptions — a cold, stomach trouble, an allergy, a migraine, or a regular medicine you forgot to bring to Japan. Medication is delivered anywhere in Japan.

What We Can Prescribe

We treat two kinds of request, and they are handled differently.

Dispensed by our clinic

Weight-loss injections

Mounjaro (tirzepatide) and Wegovy (semaglutide). We dispense and ship these ourselves by refrigerated delivery, and you pay us for the medication.

Dispensed by our partner pharmacy

General prescription medicines

Cold and fever, stomach trouble, allergy, migraine, skin problems, altitude sickness, and regular medication you forgot to bring or lost — for example blood-pressure, cholesterol, thyroid or diabetes tablets. We issue the prescription; an English-speaking pharmacist in Tokyo dispenses it, gives you a short online medication consultation, and ships it to you. You pay the pharmacy directly for the medicine.

We cannot prescribe narcotics or psychotropic drugs.

What is Online Consultation?

Online medical consultation (telemedicine) allows you to receive medical care from a Japanese physician via video call (Google Meet) — without traveling to the clinic.

After the consultation, your medication is delivered to your address in Japan. Weight-loss injections are shipped by us using refrigerated delivery. Other medicines are shipped by our partner pharmacy and normally arrive the next day.

Who Can Use Online Consultation?

  • Adults (18+) in Japan, including visitors and tourists
  • For weight-loss medication: BMI 23 or above (Wegovy: BMI 27, or 23+ with complications)
  • For general prescriptions: no BMI requirement
  • Patients comfortable with a video call (Google Meet)
  • Patients who can receive a delivery at an address in Japan, including a hotel

Languages

Consultations can be conducted in English or Japanese. Our supervising physician for online care communicates clearly in English.

Process

    Online Reservation

    Book a time slot at reserve.ikegawa-clinic.com. ¥1,000 booking fee charged to your credit card at reservation (applied to your final bill).

    None of the times work for you? You can request a specific date and time instead. Dr. Ikegawa reviews it and replies by email, usually within one hour (we reply 09:00–22:00 JST). Your card is only held — it is charged only if he accepts. A time outside our regular schedule carries an additional fee: ¥5,500 from tomorrow onward, or ¥11,000–33,000 for today depending on the hour.

    Confirmation Email

    You'll receive a confirmation email with the Google Meet URL. Please prepare ID (passport, A photo of your passport photo page is required for your first online consultation (uploaded during booking). Other IDs cannot be used in its place.

    Video Consultation

    Join Google Meet at your appointment time. Initial consultations typically take 15-30 minutes. The physician will discuss your medical history, current condition, and treatment options.

    Payment

    After consultation, you'll receive a payment email with a credit card link or bank transfer info. Payment is required within 7 days.

    Medication Delivery

    Once payment is confirmed, medication is shipped within 1-2 business days. Injectables use cool delivery (refrigerated); tablets use regular shipping.

Documents Available

  • Receipt: Issued. Use as proof of purchase.
  • Medical certificate (English): Issued on request as part of the consultation fee. As this is self-pay treatment, the certificate cannot list a specific disease name — it states the medication is for the patient's own use within the scope of self-pay treatment.

  • Prescription: For medicines dispensed by our partner pharmacy, we issue a prescription and send it to them directly. For Mounjaro and Wegovy we dispense in-house, so no prescription document is issued.

For Visitors / Tourists in Japan

Medication can be delivered to any address in Japan, including hotels. For weight-loss injections, please confirm with your hotel in advance that they can accept and refrigerate the package on your behalf.

For general prescriptions, our partner pharmacy ships to your hotel — write the hotel name, your room number and your name and the front desk can receive it for you. Same-day delivery to a hotel is also available in the Tokyo area for an additional fee.

⚠️ Customs notice — taking medication out of Japan:

From Japan's side: No special Japanese export permit is required for personal-use prescription medication. Carry the English medical certificate we issue, your receipt, and keep the medication in its original packaging.

From your destination country's side: Rules vary widely — GLP-1 receptor agonists are restricted or banned in some countries. Please verify with your destination country's embassy / customs in advance. Full country-by-country summary: see Pricing page.

Important Notes

  • You can attend the online consultation from anywhere, including overseas. However, your medication can only be delivered to an address within Japan — we cannot ship internationally, so you (or someone on your behalf) must be able to receive it in Japan.
  • Initial consultations require ID verification.
  • Online consultation is not appropriate for emergencies. For emergencies, call 119 (ambulance in Japan).
  • Prescriptions are not guaranteed based on patient request alone. The physician's medical judgment determines whether medication is appropriate.
  • Online consultations follow Japan's "Guidelines for the Appropriate Implementation of Online Medical Care" by the Ministry of Health, Labour and Welfare (MHLW).

Cancellation Policy

  • By 23:59 the day before: Full refund of ¥1,000 booking fee
  • Same-day cancellation: ¥5,000 fee (incl. tax)
  • No-show: ¥5,000 fee (incl. tax)

Frequently Asked Questions

Can I receive the consultation in English?
Yes. The internal medicine physician in charge of online care communicates clearly in English. Both English and Japanese consultations are available.
Who will I be seeing?
Your consultation is conducted by one of our internal medicine physicians, who is board-certified by the Japanese Society of Internal Medicine and communicates in English.
Do I need to be in Japan?
Not for the consultation itself — you can attend from anywhere, including overseas. However, medication can only be delivered to an address within Japan (we cannot ship internationally), so you will need a delivery address in Japan where you or someone on your behalf can receive it.
What is the 1,000 yen booking fee?
A 1,000 yen (tax incl.) fee charged when you reserve online. It is applied to your final bill on the day of the consultation. If you cancel by 23:59 the day before your appointment, it is refunded in full.
Is there a consultation fee?
Yes. For English-language consultations a 5,500 yen (tax incl.) consultation fee applies, which includes issuing a medical certificate and English-language support. The 1,000 yen booking fee is deducted from your total. For a follow-up within 3 months of your previous visit, no consultation fee applies.
What payment methods are accepted?
Credit card (Visa, JCB, Mastercard, American Express, Diners Club) and Japanese bank transfer. Payment is processed through Square Inc.'s secure system.
When will the medication be shipped?
After your payment is confirmed, we ship on the next business day or later. Our shipping days are Monday to Saturday, excluding public holidays and the year-end period from December 29 to January 3. Injectables are sent by cool delivery and usually arrive from the afternoon of the following business day.
Can I have the medication delivered to my hotel in Japan?
Yes. We can ship to any address in Japan, including hotels. Please confirm with your hotel in advance that they can accept a package on your behalf and refrigerate it. Mounjaro and Wegovy are injectables shipped by cool delivery and must be kept refrigerated. The clinic cannot take responsibility if a package is refused, returned, or spoiled because the hotel could not accept refrigerated delivery.
Can you issue a medical certificate in English?
Yes. An English medical certificate is issued on request as part of the consultation fee. Because this is self-pay treatment, the certificate cannot state a specific diagnosis; it states that the medication is for the patient's own use within the scope of self-pay treatment. A receipt is also issued. We dispense medication in-house, so a separate prescription form is not generated.
