英文診断書の料金の変更について
（2026年12月1日〜）

2026年12月1日から、英文診断書の一部を有料といたします。

• 初診で処方したお薬の診断書1通は、これまでどおり初診の診察料に含まれます。後日にご依頼いただいた場合も同じです。
• 初診の診察料に含まれる1通をまだお使いでない場合は、初診と再診の両方の処方をまとめた新しい診断書1通を、この1通として発行できます。
• それ以外の新しい診断書と、ご依頼による診断書の修正は、1通につき3,300円（税込）を頂戴します。
• 同じPDFの再送と、当院の誤りの訂正は、引き続き無料です。
• 診察時以外にご依頼いただいた診断書に料金がかかる場合は、お支払いのリンクをメールでお送りします。お支払いを確認した後、診断書をPDFにしてメールでお送りします。

2026年12月1日より前にご予約いただいた診察は、現在の取り扱いのままといたします。そのご予約の診察について発行する診断書は、診察が12月1日以降になる場合も無料です。

2026年12月1日より前に当院に届いた診断書のご依頼も、現在の取り扱いでお受けします。

English: Change to English medical certificate fees from 1 December 2026

From 1 December 2026, we will charge for some English medical certificates:

• One certificate covering your initial prescription remains included in your first-visit consultation fee, even if you request it later.
• If you have not used your included certificate, you may use it for one new certificate covering both your initial and follow-up prescriptions.
• Other new certificates, and revisions you request, will cost ¥3,300 (tax included) each.
• Resending the same PDF, and correcting a mistake made by our clinic, remain free.
• If a fee applies to a certificate you request outside a consultation, we will email you a payment link. Once your payment is confirmed, we will email the certificate to you as a PDF.

Appointments booked before 1 December 2026 keep the current terms, so a certificate issued for that appointment stays free, even if the consultation takes place on or after 1 December.

Certificate requests we receive before 1 December 2026 are also handled under the current terms.
